Medical History Form

To speed up your check-in process, create an account via our e-booking system and complete your medical history form online. Otherwise leave a extra little time to complete when you arrive for your first appointment.

Payment Policy

Unless other arrangements have been made, payment is required at each appointment. We have HICAPS for direct billing of your health insurer. We also accept cash, credit card and EFTPOS payment.

Cancellation & Missed Appointment Policy

Your appointment time is set aside for you alone. We ask that you notify us at least 24 hours in advance if you cannot attend your scheduled appointment to allow us to offer the time to another patient.

Getting to Aria Dental

Aria Dental is located at 1002 Hay Street, Perth WA 6000, not far from the QV1 building

  • We offer complimentary parking at the QV1 Public Car Park. You access the QV1 car park off Murray Street, just near Hertz Rent-a-car. You will receive a parking ticket as your drive through the entry boom gate, once you find yourself a park, bring that entry ticket to Aria reception and we will swap your ticket for a pre-paid exit ticket for after your treatment. *Please note, this is a public car park and unfortunately we have no control should the car park be ‘full’.



  • Public Transport : Aria Dental is located just across the road from the QV1 and the free RED CAT bus stop for QV1 is #14. Just jump off here, cross the road and Aria Dental is just 3 minutes’ walk. If you are catch the train, get off at the Central Perth Underground stop, under Raine Square, then jump on the free RED CAT bus or walk 20 mins to Aria Dental.  View the CAT bus map here.
